Surgical Associates of Fresno Medical Clinic, Inc.

Prior to your first appointment, you will need to complete a new Patient Form and a Medical History.  These can be completed in the office on the day of your appointment or you may print these forms and complete them prior to your visit.  This will save you time waiting in our office and will ensure that you have all the necessary information at your disposal.  You will need to bring the completed forms as well as all insurance information and cards with you to your first appointment.
